Welcome to Health & Community Services (HCS)

We are a proudly registered NDIS provider supporting individuals and families across Victoria.

Our mission is simple — to empower people of all abilities to live fulfilling, independent lives through compassionate, professional, and person-centred care.

At HCS, we believe that every journey is unique. That’s why our dedicated team of mental health and general nurses, allied health practitioners, behaviour support specialists, social workers, and experienced support workers collaborate to deliver holistic, individualised care.

Whether you’re seeking support for yourself or a loved one, our goal is to make the process simple, responsive, and supportive — with most services commencing within 72 hours of completing your service agreement.

Mental Health & Dual Diagnosis Expertise

We specialise in supporting people living with mental health conditions and dual diagnoses, where a mental health challenge may coexist with alcohol or other drug use.

Our clinicians use a recovery-focused framework built on:

• Self-determination and hope
• Personal goal-setting and capacity building
• Community inclusion and social participation
• Ongoing support and reassessment

Our mental health nurses and social workers conduct comprehensive, needs-based assessments to identify goals and risks, ensuring every support plan is purposeful and safe.

We also collaborate closely with GPs, psychiatrists, and public/private mental health services to ensure continuity of care — including medication administration (e.g., long-acting injectables) and crisis management when needed.
